Established in 1998, Desert Nursing Concierge is a branch of U Care, We Care Home Health Services, based in Laguna Niguel, California. Our nurses and healthcare team provide care to clients in a variety of settings, including private homes, assisted living communities, and hospitals.

We work closely with physicians and families to ensure that each client receives safe, effective, and personalized care.

We provide services beginning at the time of discharge from inpatient settings to support a smooth transition home. Our staff strongly believes that prevention and ongoing maintenance are essential to achieving a healthy and productive life.

• Comprehensive Nursing Assessment: One of the most important components of prevention and care. A nurse will gather essential health information, including vital signs and blood glucose levels, provide nutrition education, develop an individualized plan of care, and initiate safety measures and a medication review checklist.

• Nurse Case Management: A registered nurse will visit weekly, or as needed, to monitor medication adherence, diet, blood glucose levels, blood pressure, skin integrity, and overall safety. Services may also include administering IV medications and providing support for addiction and recovery.

• Already have a caregiver? Our nurses can collaborate with your existing caregiver or family member to ensure consistent, high-quality care at all times.

• Assisted Living & Board and Care Monitoring Services: Our nurses oversee and evaluate the care provided within assisted living and board and care facilities. In response to staffing limitations and varying levels of care quality, we conduct thorough assessments that include risk for injury, medication management, vital signs, blood glucose levels, fluid intake, appetite, and elimination patterns.
• Our goal is to ensure each client’s health is maintained—and, when possible, improved—through proactive monitoring and early intervention. Prevention is key.

• Nursing Concierge Services: Personalized, in-home care provided by a licensed nurse to closely monitor your progress. This service includes ongoing communication with your physician and family to provide updates on your condition and obtain any necessary orders to ensure optimal care.

Nursing and Caregiver Referrals: Our referral services comply with all independent contractor requirements mandated by state law. All referred contractors are responsible for maintaining their own malpractice insurance and paying applicable taxes.

Meet the Manager

Natalie Hanna is a registered nurse, board-certified by the American Nurses Credentialing Center. She works with a wide range of healthcare professionals throughout the Coachella Valley and teaches CPR and Advanced Cardiac Life Support (ACLS). Natalie attended the College of the Desert Nursing Program and has lived and worked in the area for the past 30 years.

Natalie co-founded U Care, We Care Home Health Services in Laguna Niguel and has been fortunate to serve the veteran population in the West Los Angeles area. She has extensive experience caring for individuals struggling with drug and alcohol addiction, having worked at the Betty Ford Center and served as Director of Nursing at Banyan Treatment Center.

In addition, Natalie collaborates with prominent addiction specialists and concierge physicians around the world. Her experience also includes caring for individuals dealing with depression and other mental health conditions.

Natalie and her team have now expanded their focus to include healthcare prevention and wellness, aiming to provide optimal care while helping to prevent unnecessary injuries, relapses, and hospital admissions.
